Client Forms

If you are a new client scheduling your first appointment, you will receive access to a secure client portal via your email. Within this portal, you will be instructed to complete a brief history, sign consents, provide demographic information and confirm appointment status. This digital method allows you the convenience of not having to download, complete and sign hard copies of our office documentation. After you have received your email notification, you will have 24 hours to access the link and sign in to the portal before the link expires. If you have any problems or cannot sign in within 24 hours, please call the office (832-846-6206) and we can assist you and resend another link to your attention.

This documentation is required to be completed prior to your first scheduled therapy session. Within this portal documentation, there are legal documents such as consent to treatment and limits to confidentiality are included. Further completion of the client history and intake, allows your therapist to be prepared to meet and guide you. If your digital forms are not completed prior to your first appointment, we may need to reschedule you at a more convenient time for you to be able to complete portal documentation.

If you are an existing client of Insight Life Counseling, your client portal allows you to request or change appointments, pay your bill on line and send secure messages to your therapist.
